Requirements

Magic: 50+ (Iban's Blast) or 75+ (Trident)
Prayer: 43+ (Protection Prayers)
Recommended: Morytania Hard Diary for 50% more runes

The Six Brothers

Dharok: Pray Melee. Most dangerous — can hit 60+ at low HP. Kill first
Ahrim: Pray Magic. Use melee against him (weak to melee)
Karil: Pray Ranged. Use magic against him
Verac: Pray Melee. His attacks can hit through prayer occasionally
Guthan: Pray Melee. Heals from attacks. Kill quickly
Torag: Pray Melee. Easiest brother. Slow attacks

Optimal Route

Start with Dharok (most dangerous, fresh supplies)
Ahrim → Karil → Guthan → Verac → Torag
Enter the tunnels through the remaining brother's crypt
Navigate to the chest, kill the tunnel brother, loot

Gear Setup

Weapon: Iban's Staff (u) or Trident of the Swamp
Armor: Mystic or Ahrims top/bottom, Melee switch for Ahrim
Prayer: 4-6 Prayer Potions
Food: 4-8 Monkfish or better
Teleport: Barrows Teleport tab or Morytania Legs

Maximizing Profit

Kill 880+ reward potential in the tunnels (kill bloodworms/skeletons) for maximum rune drops
Morytania Hard Diary adds 50% more runes to every chest
Trident of the Swamp speeds up kills significantly over Ibans
